SUBMITTED BY: REEVES, CHRISTOPHER 0663 (AR09111870) DID ATTEMPT TO ACQUIRE OR OBTAIN POSSESSION OF A CONTROLLED SUBSTANCE BY MISREPRESENTATION, FRAUD, FORGERY, DECEPTION OR SUBTERFUGE, IN VIOLATION OF FLORIDA STATE STATUTE 893.13(7)(A)9. ON 111109 AT APPROXIMATELY 1437 HOURS, I WAS DISPATCHED TO 9427 SOUTH SUNCOAST BOULEVARD IN HOMOSASSA IN REFERENCE TO A FRAUD WHICH WAS IN PROGRESS. DISPATCH ADVISED THAT THE PHARMACISTS AT THIS ADDRESS, WHICH IS HOMOSASSA WALGREEN'S, ADVISED THE DEFENDANT, MS JODIE LANIER, HAD GIVEN THE PHARMACY OBVIOUSLY FRAUDULENT PRESCRIPTIONS AS WELL AS AN INSURANCE CARD. ON ARRIVAL, I MADE CONTACT WITH THE PHARMACISTS WHO ADVISED THAT THE DEFENDANT HAD TURNED IN TWO PRESCRIPTIONS, ONE BEING OXYCODONE 60 COUNT OF 30 MILLIGRAM TABLETS, AND ONE BEING FLEXERIL 30 COUNT OF 10 MILLIGRAM TABLETS. THE PHARMACISTS ADVISED THAT BOTH THE DOCTOR AND THE PATIENTS NAMES WERE NON-EXISTENT. HE ALSO ADVISED THAT THE DEFENDANT CALLED THE PHARMACY FROM TELEPHONE NUMBER OF 352-503-7723, WHICH WAS LISTED AS THE DOCTOR'S TELEPHONE NUMBER WHERE SHE REPRESENTED HERSELF AS THE PATIENT ON THE FRAUDULENT PRESCRIPTION INQUIRING AS TO HOW MUCH THE TOTAL AMOUNT FOR THE PRESCRIPTION WOULD BE. THE PHARMACISTS ALSO ADVISED THAT THE DEFENDANT HAD DROPPED OFF TWO PRESCRIPTIONS AT THE WALGREEN'S AT US HIGHWAY 19 AND GROVER CLEVELAND BOULEVARD IN HOMOSASSA. ONE PRESCRIPTION BEING FOR OXYCODONE 90 COUNT 30 MILLIGRAM TABLETS, ONE BEING FOR FLEXERIL 60 COUNT OF 10 MILLIGRAM TABLETS. THE PRESCRIPTIONS THAT WERE DROPPED OFF AT THE US HIGHWAY 19 AND GROVER CLEVELAND BOULEVARD WALGREEN'S BOTH HAD THREE REFILLS EACH. THE PRESCRIPTIONS DROPPED OFF AT THE ORIGINAL INCIDENT LOCATION HAD REFILLS OF TWO FOR EACH PRESCRIPTION. THE PHARMACISTS ADVISED THAT ON THESE TYPES OF PRESCRIPTIONS, NO REFILLS ARE ADMINISTERED. THE PHARMACISTS ALSO ADVISED THAT HE KNEW THAT THE PRESCRIPTIONS WERE FAKE DUE TO THE FOLLOWING REASONS: THE AD NUMBER ON ALL FOUR PRESCRIPTIONS ONLY HAD SIX NUMBERS. THE PHARMACISTS ADVISED THAT IT SHOULD HAVE SEVEN IF IT WERE A LEGITIMATE PRESCRIPTION. HE ALSO ADVISED THAT THE PAPERS APPEARED TO BE PRINTED OFF OF A REGULAR WHITE PIECE OF PAPER. THE PHARMACIST ALSO ADVISED THAT THE DOCTOR FOR THE ADDRESS GIVEN WAS HAAN QUINTIN WITH AN ADDRESS OF 1386 SOUTH SUNCOAST BOULEVARD IN HOMOSASSA WITH THE DEFENDANT'S HOME TELEPHONE NUMBER AS THE DOCTOR OFFICES TELEPHONE NUMBER. HE ADVISED THAT THIS DOCTOR DOES NOT EXIST. I RAN AN NCIC/FCIC CHECK AS WELL AS A CHECK IN "D.A.V.I.D." WHICH CONFIRMED THAT THIS PERSON DID NOT EXIST. THE PATIENTS NAME ON THE PRESCRIPTIONS IS MS TAMMY TURNER WITH AN ADDRESS OF 1695 AUTOMN STREET IN HOMOSASSA FLORIDA 34448. AN NCIC/FCIC CHECK WAS RUN AS WELL AS A "D.A.V.I.D." CHECK, WHICH ALSO RESULTED IN ME DISCOVERING THAT THIS PERSON DID NOT EXIST. ALSO THE AUTOMN STREET IN HOMOSASSA WAS SPELLED INCORRECTLY ON THE PRESCRIPTIONS. CONTACT WAS MADE WITH THE DEFENDANT AT THE ORIGINAL INCIDENT LOCATION, WHERE I EXPLAINED TO HER WHO I WAS AND THE REASON I WAS SPEAKING WITH HER. AT THAT TIME, I READ THE DEFENDANT HER MIRANDA RIGHTS VIA MY AGENCY ISSUED CARD, AND ASKED HER IF SHE UNDERSTOOD HER RIGHTS, TO WHICH SHE REPLIED THAT SHE DID. I ASKED HER IF SHE WISHED TO SPEAK WITH ME, TO WHICH SHE REPLIED THAT SHE WOULD. WHEN PRESENTED THE INFORMATION I HAD RECEIVED FROM THE PHARMACISTS IN REFERENCE TO THE PRESCRIPTIONS BEING FAKE, THE DEFENDANT ADVISED THAT ON 111009 SHE HAD PRINTED ALL FOUR PRESCRIPTIONS ON HER COMPUTER AT HOME AND THAT SHE HAD MADE BOTH THE DOCTORS AND THE PATIENTS NAMES UP. THE DEFENDANT ADVISED THAT WHEN SHE ATTEMPTED TO PICK UP THE PRESCRIPTIONS AT THE WALGREEN'S ON US HIGHWAY 19 AND GROVER CLEVELAND ON THE NIGHT OF THE TENTH THAT SHE HAD NOT BEEN ABLE TO DO SO. SO SHE TRIED THE WALGREEN'S AT THE ORIGINAL INCIDENT LOCATION. SHE FURTHER STATED THAT SHE DID NOT INTEND TO SELL THE PRESCRIPTIONS THAT SHE WAS MERELY ADDICTED TO THEM AND WISHED TO USE THEM FOR HER PERSONAL CONSUMPTION. IT SHOULD ALSO BE NOTED THAT THE FRAUDULENT DOCTORS SIGNATURE MATCHES THE SIGNATURE ON THE DEFENDANT'S LICENSE. I THEN CALLED THE NUMBER FROM THE PRESCRIPTION WHICH THE DEFENDANT ADVISED WAS HER HOME TELEPHONE NUMBER AND ACTUALLY SPOKE WITH HER HUSBAND. ALL PARTIES INVOLVED COMPLETED SWORN WRITTEN STATEMENTS WHICH WILL BE TURNED IN TO RECORDS. THE FRAUDULENT PRESCRIPTIONS WILL ALSO BE TURNED IN TO RECORDS ALONG WITH A FRAUDULENT PRESCRIPTION DISCOUNT CARD THAT THE DEFENDANT ATTEMPTED TO USE IN PURCHASING THE PRESCRIPTIONS. THE DEFENDANT WAS ARRESTED AT THIS TIME, HANDCUFFED (DOUBLE LOCKED) BEHIND HER PERSON, SECURED IN THE REAR SEAT OF MY PATROL VEHICLE, TRANSPORTED TO THE CITRUS COUNTY DETENTION FACILITY WITHOUT FURTHER INCIDENT. THE DEFENDANT WAS CHARGED WITH TWO COUNTS OF ATTEMPTING TO OBTAIN A CONTROLLED SUBSTANCE BY FRAUD. BOND WAS SET AT $10,000 TOTAL PER THE BOND SCHEDULE. *NOT-EXEMPT* |
