SUBMITTED BY: ALEXANDER, DAWN 0561 (AR11120136) DID ACQUIRE OR ATTEMPT TO ACQUIRE A CONTROLLED SUBSTANCE, TO WIT: VICODIN HP 10/660, BY MISREPRESENTATION, FRAUD, FORGERY, DECEPTION OR SUBTERFUGE, IN VIOLATION OF FLORIDA STATUTE 893.13(7)(A)9. ON 040611 I RESPONDED TO THE CRYSTAL RIVER SUBSTATION AND MADE CONTACT WITH DEPUTY DERA. HE ADVISED that HE HAD RESPONDED TO DOCTOR CORBETT'S OFFICE IN REFERENCE TO PRESCRIPTION FRAUD. UPON ARRIVAL, THE WITNESS ADRIANNE HESS, STATED THAT THEY HAD RECEIVED A CALL FROM WINN DIXIE LOCATED AT 333 HIGHLANDS AVENUE IN INVERNESS IN REFERENCE TO A STEPHANIE HARPER and HER PRESCRIPTIONS. WINN DIXIE HAD CALLED DOCTOR CORBETT'S OFFICE IN REFERENCE TO OBTAINING A PRESCRIPTION NUMBER AND THEY WERE ADVISED THEY DID NOT AUTHORIZE THESE PRESCRIPTIONS. THEY IDENTIFIED THE PRESCRIPTIONS BY THE PRESCRIPTION NUMBER, WHICH WAS 0004117. WINN DIXIE THEN FAXED DOCTOR CORBETT'S OFFICE THE FIVE PRESCRIPTIONS THAT WERE ALL WRITTEN IN STEPHANIE HARPER'S NAME. THE WITNESS ADRIANNE HESS STATED THAT THESE WERE NOT AUTHORIZED PRESCRIPTIONS, THEY WERE FRAUDS. THE ORIGINAL PRESCRIPTION DATED 072910 WAS ORIGINALLY WRITTEN FOR VICODIN HP, QUANTITY OF 120, WITH A REFILL DATE OF 081010. THE DEFENDANT HARPER CHANGED THE 10 TO A 1 SO SHE COULD GET THE PRESCRIPTION EARLIER. THEN ON 111210 SHE PASSED A PRESCRIPTION FOR VICODIN HP 10/660, IN THE AMOUNT OF 180 TABLETS, WHICH THE DOCTOR DID NOT PRESCRIBE, USING THE SAME PRESCRIPTION PAD SHE PASSED ANOTHER FORGED PRESCRIPTION ON 121510 FOR VICODIN HP 10/660, IN THE AMOUNT OF 180 TABLETS. ON 012111 SHE AGAIN PASSED A FRAUDULENT PRESCRIPTION FOR VICODIN HP 10/660 NUMBER 180 AND THEN ON 022111 SHE PASSED HER FINAL SCRIPT FOR VICODIN HP 10/660 IN THE AMOUNT OF 180 TABLETS. WE MADE CONTACT WITH STEPHANIE HARPER AND ASKED IF she would RESPOND TO THE CRYSTAL RIVER SUBSTATION TO SPEAK WITH US. SHE ADVISED SHE WOULD. UPON HER ARRIVAL SHE WAS READ HER MIRANDA VIA AN AGENCY ISSUED PRE-PRINTED CARD AND AGREED TO SPEAK WITH US. SHE STATED that SHE DID PASS THESE FRAUDULENT PRESCRIPTIONS AND SHE ONLY DID IT BECAUSE SHE HAD CANCER LAST YEAR and THAT THE PAIN MEDICATION WAS NOT WORKING FOR HER and she NEEDED MORE, TO EASE HER PAIN. SHE ALSO ADVISED that SHE HAD TO TAKE MORE PAIN MEDICINE THAN PRESCRIBED AND THAT IS WHY SHE WAS RUNNING OUT HALFWAY THROUGH THE MONTH. SHE STATED THAT SHE DID NOT WANT TO BE LIKE A CRIMINAL AND GO OUT and BUY THEM ON THE STREET. I ADVISED DEFENDANT HARPER THAT SHE WOULD BE PLACED UNDER ARREST FOR FIVE COUNTS OF PRESCRIPTION FRAUDS. I DID ADVISE HER THAT I WOULD RELEASE HER ON HER OWN RECOGNIZANCE, DUE TO THE FACT OF HER ALREADY BEING OUT ON A $20,000.00 BOND FOR OTHER PRESCRIPTION FRAUD. (SEE CASE NUMBER 11031164). MS HARPER IS TRYING TO GET HELP FOR HER ADDICTIONS TO THE PAIN PILLS. SHE STATED THAT THESE WERE THE ONLY OUTSTANDING PRESCRIPTIONS LEFT AND THAT THERE WOULD BE NO MORE. MS HARPER WAS HANDCUFFED (DOUBLE LOCKED) and TRANSPORTED TO THE CITRUS COUNTY DETENTION FACILITY WHERE SHE WILL BE RELEASED ON HER OWN RECOGNIZANCE. A STATEMENT FORM WAS COLLECTED FROM WITNESS HESS, ALONG WITH COPIES OF THE FRAUDULENT PRESCRIPTIONS AND ALL WILL BE TURNED IN TO RECORDS.
